PeopleTrack now offers customizable web forms in order to expand the options for our clients looking to have their clients enroll or renew their membership from anywhere. Forms allow the entire process to be completed from any device while seamlessly integrating with our visitor and membership management systems.
During the implementation process, our team will work with you to tailor the form to meet your requirements, brand it with your logo, and customize the color, theme, style, and layout options to fit within your existing or desired web design. The standard input fields include: Name, Date of Birth, Gender, Email, Class, Phone Number, Emergency Contact, Address, Photo, Document Upload, Electronic Signature, and the ability to add additional Family Members.
Our Flexforms can be embedded directly into your website, social media page, accessed via QR code, or by private link. Submissions are uploaded to your PeopleTrack database assuring new enrollments will be input into the system and ready to check-in when they arrive!
Submission email notifications can be sent to the user, or to the administrator for approval, with an attached copy of the completed document.
